Position: Full-Time RN Case Manager (RN) | Symbii Home Health & Hospice | Chubbuck, ID

Qualifications

Registered Nurse (RN) licensed in the State of Idaho.
Valid Driver's License.
Home Health and Hospice experience preferred.

Responsibilities

Completes an initial assessment of patient and family to determine home care needs and provides a comprehensive assessment and history of current and prior illnesses.

Regularly re‑evaluates patient nursing needs.

Initiates the plan of care and revises it as patient status and needs change.

Uses health assessment data to determine nursing diagnoses.

Develops a care plan establishing goals based on nursing diagnoses and incorporating therapeutic, preventive, and rehabilitative nursing actions, involving the patient and the family in the planning process.

Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed by the physician.

Counsels the patient and family in meeting nursing and related needs.

Provides health‑care instructions to the patient as appropriate per assessment and plan of care.

Identifies discharge‑planning needs and implements them before the patient is discharged.

Acts as case manager when assigned by the Clinical Supervisor or designee and coordinates patient care for the assigned caseload.

Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct.

Communication

Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains and receives physician orders as required.
Communicates with community health‑related persons to coordinate the care plan.

Additional Duties

Participates in on‑call duties as defined by the on‑call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
