Position: Full-Time RN Case Manager (RN) | Symbii Home Health & Hospice | Chubbuck, ID

Qualifications

  • Registered Nurse (RN) licensed in the State of Idaho.
  • Valid Driver's License.
  • Home Health and Hospice experience preferred.
Responsibilities
  • Completes an initial assessment of patient and family to determine home care needs and provides a comprehensive assessment and history of current and prior illnesses.
  • Regularly re‑evaluates patient nursing needs.
  • Initiates the plan of care and revises it as patient status and needs change.
  • Uses health assessment data to determine nursing diagnoses.
  • Develops a care plan establishing goals based on nursing diagnoses and incorporating therapeutic, preventive, and rehabilitative nursing actions, involving the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health‑care instructions to the patient as appropriate per assessment and plan of care.
  • Identifies discharge‑planning needs and implements them before the patient is discharged.
  • Acts as case manager when assigned by the Clinical Supervisor or designee and coordinates patient care for the assigned caseload.
  • Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct.
  • Communication
    • Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
    • Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains and receives physician orders as required.
    • Communicates with community health‑related persons to coordinate the care plan.
    Additional Duties
    • Participates in on‑call duties as defined by the on‑call policy.
    • Ensures that arrangements for equipment and other necessary items and services are available.
