Direct Support Lead

Responsibilities

• Provide person-centered direct support to individuals with developmental or behavioral needs in home and community settings
• Lead and mentor direct support staff, modeling best practices in care and documentation
• Implement individualized care and support plans in collaboration with clinical and care teams
• Monitor individuals' progress, safety, and well-being, escalating concerns as needed
• Coordinate daily activities, community outings, and skill-building opportunities
• Ensure compliance with health, safety, and regulatory standards
• Maintain accurate, timely documentation and reports in line with company policies
• Communicate effectively with families, guardians, and interdisciplinary team members
