Job DescriptionSan Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams l’s PACE provide a hands‑on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
PositionFull‑time Physician Assistant or Nurse Practitioner – Monday to Friday at our PACE clinic in Chula Vista, CA.
Job SummaryUnder the clinical direction and supervision of the Medical Director, provides primary care to participants. Performs physical assessments, develops and implements appropriate plans of care for St. Paul's PACE program participants. Evaluates physical complaints and provides appropriate treatment; provides participant and caregiver teaching and education. Participates as a member of the Interdisciplinary Team, demonstrating the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and PACE standards.
Qualificationsand Requirements
- Education: Graduate of a professional physician assistant or nurse practitioner school. Licensed in the State of California and certified by a national certifying organization recognized by the State.
- Experience: Minimum of one year working with the frail and elderly population. Two years as a physician assistant or nurse practitioner in a geriatric setting is desirable.
- Valid California license as a Physician Assistant or Nurse Practitioner.
- Degree from an NCCPA‑certified program.
- DEA Number.
- Clear DOJ and FBI fingerprint clearance and drug screen clearance.
- Thorough knowledge of current concepts, theories, and practices related to home and community‑based care for the elderly and disabled adults.
- Knowledge of PACE regulations.
- Understanding of the physical, mental and social needs of frail older adults.
- Ability to lead and work within an interdisciplinary setting.
- Capability to manage changing priorities per needs of the PACE program and the agency.
- Flexible vacation time – 17 days.
