LVN, LPN​/LVN, RN Nurse

Aveanna Healthcare is seeking Licensed Vocational Nurses to provide compassionate in-home care for pediatric and adult patients during night and weekend AM shifts. In this role, you will deliver skilled nursing services, including medication administration, treatments, and vigilant monitoring of patient conditions, following physician-directed care plans. You will collaborate closely with an interdisciplinary team, educate families on home care techniques, and maintain accurate clinical documentation.

Responsibilities

• Provide direct nursing care to patients in their homes during night and weekend AM shifts
• Administer medications and treatments per physician orders and care plans
• Monitor vital signs, symptoms, and patient responses, documenting accurately in clinical records
• Collaborate with RNs, physicians, therapists, and caregivers to coordinate patient care
• Educate patients and families on disease management, medications, and home care techniques
• Follow infection control, safety, and clinical protocols to ensure high-quality care
• Report changes in patient condition promptly and participate in care plan updates
• Maintain accurate, timely documentation and required reports
• Use medical equipment and technology safely and effectively in the home setting
