Position: LPN / LVN

Sevita is seeking a compassionate LPN / LVN to deliver person-centered nursing care in home and community-based settings. In this role, you'll provide direct care, including medication administration, treatments, and health monitoring for individuals with behavioral health needs, brain injuries, and other complex conditions. You'll collaborate with an interdisciplinary team, support individualized care plans, and maintain accurate clinical documentation. Responsibilities

  • Provide direct nursing care, including medication administration and treatments, under physician and RN direction
  • Monitor, document, and report changes in patient condition and vital signs
  • Support individualized care plans for individuals with behavioral health needs, brain injuries, and other complex conditions
  • Educate individuals and families on health management, medications, and care routines
  • Maintain accurate, timely clinical documentation in accordance with regulatory and company standards
  • Collaborate with an interdisciplinary team to coordinate services and ensure person-centered care
  • Ensure compliance with safety, infection control, and quality standards in home and community settings
  • Assist with daily living activities, health monitoring, and crisis intervention as needed
  • Participate in ongoing training, competency checks, and professional development programs
  • Communicate with families, caregivers, and external providers to support continuity of care
