We are seeking a dedicated and compassionate Registered Nurse (Home Health) to join our client’s growing team in Cicero, Illinois. This role focuses on delivering skilled, patient-centered care in the comfort of patients’ homes, helping them maintain independence and improve overall well-being. The ideal candidate will bring clinical expertise, strong communication skills, and a genuine commitment to providing high-quality care.

• Deliver professional nursing care and perform comprehensive patient assessments in home settings
• Develop individualized care plans in collaboration with physicians and the healthcare team
• Educate patients and families on treatment plans, medications, and post-care instructions
• Monitor patient progress and adjust care as needed to achieve optimal outcomes
• Maintain accurate and timely documentation in accordance with regulatory standards
• Ensure effective communication and coordination among care team members

• Monday through Friday, day shift
• 1 weekend per month
• 2‑3 on-call days per month and 2 holidays per year
• This is a Pay Per Visit position

• Local travel required within the Cicero area (travel reimbursement provided)

• Associate degree or higher in Nursing
• Active Registered Nurse (RN) license in Illinois
• Minimum of 1 year of professional RN experience (home health preferred)
• Current BLS and CPR certification through the American Heart Association
• Valid driver’s license, reliable vehicle, and current auto insurance

• Prior experience in home health or community-based nursing
• Strong assessment, communication, and time management skills
