Overview
The successful candidate will assist the Director of Medical Services in the delivery of appropriate medical services and supports to adult participants served by UCP Seguin of Greater Chicago, ensuring the appropriate coordination of delivery of all health services in accordance with each participant's service plan and needs, and with regulations and standards governing such services as established by state and federal regulatory agencies, applicable laws and accreditation organizations;
ensure UCP Seguin adult staff are trained appropriately in methods and techniques related to self-administration of medication program and other applicable medical care regimes/programs and the process is monitored sufficiently to ensure effectiveness.
Assists the Director of Medical Services, as assigned, with the provision of medical services to persons served in the Adult Program which includes but is not limited to the following:
- Trains DSP staff on Medication Administration, on-the-job training (OJT), conducts Nursing Assessments, (Self Administration of Medication Assessments (SAMAs), and Health Risk Screening Tool (HRST).
- Secures, maintains and updates medical, dental, vision, medication and immunization histories in collaboration with client physicians, families, and the Provider Support Team (PST) for all UCP Seguin clients and those seeking admission to agency programs;
- Coordinates/provides monitoring and evaluation of each assigned participant's medical needs and care; making recommendations for additional medical services/alternative and/or augmentative services, following up as needed to ensure the health/welfare of persons served;
Participates as a member of the PST as appropriate in Implementation Strategy (IS) meetings, providing relevant medical information and input in the development of the IS in collaboration with the participants physician; prepares reports as needed, summarizing client services and needs for presentation at IS meetings for persons served; - Functions as a liaison between each participant's medical practitioner in collaboration with the PST to ensure timely and appropriate changes in each participant's medical care and communicating changes to the PST, family members, guardians, and any other personnel supporting the participant.
- Functions as a liaison in collaboration with the PST between the selected health care organizations (hospitals, clinics, home health and visiting nurses, etc.) providing all routine and emergency care for participants served by UCP Seguin.
- Completes Agency training / certification requirements and secures additional information, education, and training appropriate to remain abreast of new knowledge, technologies and methodologies to ensure understanding of concepts and principles upon which work responsibilities are based and acquire requisite skills to perform job duties effectively.
- Shares rotating on-call responsibilities with the other RNNTs at UCP Seguin;
- Participates as a Medical Services team member, facilitating communication, teamwork, cooperation and productivity of work unit by:
- Attending and actively participating in regular work unit, department and Agency staff, program meetings and assigned ad hoc committees, offering/receiving information, constructive assessments, feedback and suggestions to ensure cross-team collaboration, service integration and maximize operational effectiveness and participant performance;
- Participating in constructive problem solving within the work unit and as it relates to other Agency programs, adults served, their families, staff, and the community, in order to ensure successful work unit and Agency operations, and a positive, productive and harmonious work environment;
- Remaining abreast and acting on information posted in work unit, department, and Agency information posted and conveyed by way of internal Agency communication resources.
