Intake Coordinator

Care Core Health is seeking an organized, detail-oriented Central Intake Coordinator to support the referral and admissions process across multiple skilled nursing facilities in the Cincinnati Reigon. This role is an important part of our centralized intake team and works closely with Business Development, Marketing, and facility leadership to ensure referrals move efficiently from initial review through admission.

• Manage and process new referrals in GatewAI for multiple Care Core facilities
• Verify insurance eligibility, coverage, network status, and authorization requirements
• Initiate, track, and follow insurance authorizations through completion
• Monitor referrals for updates, documentation, and potential barriers to admission
• Ensure required admission documentation, including HENS/PASRR and Medicaid Pending requirements is completed
• Prepare and maintain accurate admission documentation in Point Click Care  (PCC)
• Communicate insurance findings, missing information, and admission barriers to Marketing and facility teams
• Prepare daily admission reports and new admission communications
• Cross-train across Care Core facilities and provide team coverage as needed
• Participate in an assigned holiday coverage rotation

• Strong organizational and time-management skills
• Ability to manage multiple facilities, referrals, and competing priorities
• Excellent attention to detail and documentation accuracy
• Professional written and verbal communication skills
• Comfortable learning and working within referral, insurance, authorization, and electronic medical record systems
• Knowledge of healthcare insurance verification and authorization processes, or the ability to learn quickly
• Ability to work independently while collaborating with a centralized team
• Strong follow-through and ability to recognize and elevate barriers appropriately

• Experience in skilled nursing, post-acute care, admissions, intake, healthcare business development support, insurance verification, authorization, or a related healthcare setting
• Experience working with Medicare, Medicaid, Managed Care, and commercial insurance
• Experience with Point Click Care  (PCC) and/or electronic referral management systems is strongly preferred
