PrimaryCare Physician (PCP) Overview
- Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.
- Coaching for health: The PCP acts as a health coach, helping patients set short and long‑term health goals, partnering with the patient to work toward those goals, and frequently following up on progress toward improved health.
- Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient and for leading care teams toward performance goals.
We are an outcomes‑focused, value‑based organization. For each PCP’s panel of patients, metrics such as patient admissions per thousand, use of 18‑21 appointment slots per day, CGCAHPS (patient experience), clinical gaps closures, and medical cost measures are regularly measured. Each PCP will have goals for these metrics and will work toward these targets with center and market leadership and care teams.
