Care Physician (PCP) Overview
  • Accountability for outcomes: The PCP demonstrates accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of up to 450 patients. They understand that they can strongly influence the patient’s outcomes by building a trusting relationship and helping them change behaviors.
  • Coaching for health: The PCP acts as a health coach, helping patients set short and long‑term health goals, partnering with the patient to work toward those goals, and frequently following up on progress toward improved health.
  • Simplifying for action: The PCP simplifies and prioritizes appropriately so that behavior change is more actionable, both for the patient and for leading care teams toward performance goals.

We are an outcomes‑focused, value‑based organization. For each PCP’s panel of patients, metrics such as patient admissions per thousand, use of 18‑21 appointment slots per day, CGCAHPS (patient experience), clinical gaps closures, and medical cost measures are regularly measured. Each PCP will have goals for these metrics and will work toward these targets with center and market leadership and care teams.

Job Duties / Responsibilities
  • The PCP independently provides care for patients with acute and chronic illnesses in older adults.
  • The PCP takes full accountability for patient care and outcomes and seeks consultation from specialists when needed, while remaining involved in detailed care.
  • The PCP engages with the hospitalist whenever a patient is hospitalized, regardless of hospitalist affiliation.
  • The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs.
  • The PCP leads a care team consisting of a care promoter, care facilitator, and care coordinator for office‑based patients.
  • For patients unable to come to the office, the PCP engages with the transitional care team and other resources such as case managers and acute/transitional‑care physicians.
  • The PCP plays an active role in center management, covers for other providers as needed, and assists in recruiting patients and additional providers.
  • The PCP collaborates with an Advanced Practice Practitioner (APP) and remains accountable for the APP’s actions while employed with the company.
  • Performs other duties as assigned and modified at the manager’s discretion.

  • Physician Leadership: Continuously develop leadership skills for the benefit of patients, teams, and the company.

  • Influence: Competently and compassionately influence patients and teams to achieve the best outcomes.
  • Self‑Care: Maintain physical, mental, social, and spiritual wellness to care for others sustainably.
