We are seeking a Primary Care Physician (MD/DO) to deliver comprehensive adult and family medicine care while overseeing Advanced Practice Providers (APPs) within a primary care service line. This role combines clinical duties, leadership, and care coordination, focusing on Chronic Care Management (CCM), Transitional Care Management (TCM), and Remote Patient Monitoring (RPM) programs.
Key responsibilities include providing high-quality care, supervising APPs, supporting CCM, TCM, and RPM programs, participating in quality initiatives, and collaborating on workflows to improve patient experience. Candidates should have MD/DO with Board Certification/Eligibility in Family or Internal Medicine, an active Ohio medical license, and experience with APP collaboration preferred. Strong communication and leadership skills are essential.
