- $180,000 to $225,000+ Base Salary, depending on experience

COMPANY PROFILE:
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 40 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.
POSITION
DESCRIPTION:
The physician serves as the primary care provider for a dedicated panel of geriatric patients.
Patients are located in long-term care facilities (ALFs / ILFs) and private homes. Physicians are provided a convenient geographic area in which all patients are located and there is flexibility in regard to whether house call patients are included in the panel.
Physicians see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs. Dedicated schedulers put together daily schedules / routes for the providers to ensure efficiency and appropriate time for every visit.
Specific duties of the role include, but are not limited to:
- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and responses to treatment
- adjust treatment plans as necessary
- perform minor procedures and wound care as required (and as settings permit)
- educate patients and families on health management, disease prevention, etc.
- provide guidance on lifestyle modifications and self-care practices
- coordinate with nurse practitioners, nurses, and other healthcare professionals
The practice uses a mobile-based EHR with AI functionality to reduce administrative time.
On-call is phone triage only with very low volume and providers rotate the on-call responsibility.
REQUIREMENTS:
- Ohio medical license
- DEA
- Board Certified or Eligible, Internal / Family / Geriatric Medicine
