The Health Care Connection (THCC) was Ohio's first Federally Qualified Health Center (FQHC). Our mission is to provide quality and accessible primary healthcare services through community responsive approaches that address financial, geographic, and other barriers to care for residents of northern Hamilton County and surrounding areas. Our integrated model includes Primary Care, Pediatrics, Infectious Disease, Substance Use Treatment, Integrated Behavioral Health, Dental Services, Women's Health, and Pharmacy. The Health Care Connection is seeking a Board-Certified Pediatrician to join our team and provide high-quality, accessible primary care to children and families in the communities we serve. The Pediatrician provides direct medical care to pediatric patients in a primary healthcare setting. Responsibilities include patient assessment, diagnosis, treatment planning and implementation, evaluation, and ongoing monitoring of each patient's overall care plan.
Key Responsibilities:- Provide comprehensive primary and preventive pediatric care.
- Perform patient histories, physical examinations, assessments, and diagnoses.
- Develop and implement individualized treatment plans.
- Evaluate patient progress and modify treatment plans as appropriate.
- Monitor the overall management of assigned patients.
- Provide health education, preventive care, immunizations, and anticipatory guidance.
- Maintain accurate, timely, and confidential medical records.
- Collaborate effectively with physicians, advanced practice providers, nurses, behavioral health professionals, and other members of the care team.
- Provide culturally sensitive care to patients and families from diverse educational, socioeconomic, and ethnic backgrounds.
- Participate in shared pediatric call coverage.
- Medical degree.
- Board certification in Pediatrics.
- Completion of three years of pediatric residency training.
- Demonstrated clinical skills.
- Ability to work effectively as part of a team across the organization.
- Ability to maintain confidentiality and appropriately handle sensitive information.
- Ability to communicate effectively and build relationships with individuals from diverse educational, socioeconomic, and ethnic backgrounds.
- One to three years of experience in an ambulatory care setting, community health center, FQHC, or similar practice environment.
