This is an on-site full-time or part-time role for a Community Paramedic based in multiple markets. The Community Paramedic will provide urgent and emergent care to patients in their homes, operating under physician-directed protocols and leveraging telehealth tools. Responsibilities include conducting physical assessments, administering first-line treatments, coordinating with physicians for advanced directives, and ensuring continuity of care for patients. The Community Paramedic will also engage in care coordination, maintain accurate medical records, and provide consistent, patient-centered support.
Qualifications- Certified Paramedic license and experience in urgent care, emergency services, or a related field
- Strong clinical assessment, treatment, and critical thinking skills for patient care in a home environment
- Proficiency with telehealth technology and familiarity with electronic medical record (EMR) systems
- Outstanding communication and interpersonal skills to build trust with patients, families, and healthcare providers
- Ability to work effectively both independently in a patient’s home and collaboratively within a care team
- Flexibility to respond promptly to urgent situations and adapt to dynamic patient needs
- Valid driver’s license and reliable transportation
- BLS, ACLS, and PALS certifications are strongly preferred
- Experience working with diverse populations and addressing social determinants of health is a plus
