The Care Coordinator establishes professional relationships with guardians and families, completes referrals, and locates community supports and resources. The Coordinator works with multi-system youth and families to build independence at home, advocates for family desires, and maintains documentation that meets Medicaid compliance requirements.

QualificationsRequired: High School diploma or equivalent.
Associates in Human Services or related field is preferred.
Experience: One (1) year of work with children and/or families in areas such as family systems, community systems and resources, case management, child and family counseling/therapy, child protection or child development.
Ability to manage complex cases and navigate state and local child-serving systems.
Valid driver’s license with acceptable driving record, vehicle insurance, and proof of insurance.
- State Police, FBI, and Child Abuse Clearances; CPSL Mandated Reporter-Recognizing and Reporting Child Abuse training per state regulation.

Community, home, school; daily access to a private insured vehicle during work hours. - Working Hours:
