Summary

The Clinical Pharmacy Specialist (CPS) works under the supervision of the Director of Pharmacy, providing specialized clinical pharmacy services to patients in Respiratory care, collaborating with interdisciplinary team members to resolve drug distribution problems and achieve patient‑centered outcomes.

Qualifications
  • Master's degree (minimum), PharmD preferred.
  • Completion of an ASHP‑accredited PGY1 pharmacy residency with preference for PGY2 or fellowship.
  • Significant research contributions and medication use evaluation activity in specialty area preferred.
  • Recognition as an expert in specialty field through service, presentations, publications.
  • Registered pharmacist or eligible for licensure in Ohio.
  • 1 year equivalent experience preferred.
Salary

Expected starting salary range: $61.42/hr – $73.74/hr, dependent on experience and other factors.

Responsibilities
  • Engage in population‑appropriate communication and provide clear instructions to patients/families.
  • Involve family/guardian in assessment, initial treatment and ongoing care.
  • Identify physical limitations and deploy interventions.
  • Respond to behavioral health issues, abuse or neglect signs.
  • Interpret population data and plan care appropriately.
  • Evaluates drug therapy for appropriateness, effectiveness, safety, adherence, and affordability.
  • Clarify pharmacotherapy plans and communicate changes to prescribers and other stakeholders.
  • Monitor and adjust therapy to achieve optimal patient outcomes.
  • Analyze laboratory, exam and other data to support therapy decisions.
  • Develop therapeutic plans and address medication‑related problems through consultations and collaborative agreements.
  • Provide expertise in pharmacokinetics, therapeutic drug monitoring, evidence‑based selection, and regulatory performance.
  • Ensure continuity of care for specific patients or teams during hospitalization or disease phases.
  • Maintain on‑call responsibilities, develop ordersets, protocols, guidelines, clinical pathways.
  • Review and provide feedback on medication care delivered in absence.
  • Educate patients regarding medication use.
  • Participate in rapid response, medical emergency response and drug distribution activities during staffing shortages.
  • Adhere to formulary and pharmacy policies.
  • Demonstrate effective verbal/nonverbal communication with patients and healthcare practitioners.
  • Apply depth of pharmacology, pharmacotherapy and pathophysiology knowledge in specialty practice.
  • Maintain up‑to‑date pharmacotherapy knowledge via BPS certification (where applicable).
  • Acquire and analyze scientific evidence for evidence‑based decisions.
  • Participate in multidisciplinary teams for protocol development and continuous quality improvement.
  • Report medication incidents, adverse drug events and lead MUE initiatives.
  • Document clinical activities for dashboards and evidence of services.
  • Lead development of new clinical services for quality and cost improvement.
  • Resolve drug distribution problems identified by management, nurses and support staff.
  • Contribute to drug policy development committee evaluations.
  • Supervise and mentor pharmacists, students and residents in patient care areas.
  • Provide patient safety initiatives, transition‑of‑care improvements, cost‑reduction strategies.
  • Attend pharmacy staff meetings, support coverage, deliver educational sessions.
  • Build relationships with external professionals and allied health staff.
  • Provide education to pharmacy personnel, students and non‑pharmacy personnel.
  • Develop APPE rotation descriptions and precept at least five APPE students per academic year.
  • Represent the department in communications and professional committees.
  • Develop formal APPE descriptions; serve as project preceptor for residents.
  • Engage in research protocol development, execution, analysis and collaboration.
  • Assist with MUEs and pharmacoeconomic studies.
  • Represent pharmacy specialization at professional organizations and national associations.
  • Assume other clinical personnel roles during absences.
  • Perform other related duties as assigned by the Director of Pharmacy Services.
