The Health Care Connection (THCC) is seeking a School-Based Behavioral Health Provider for a unique opportunity to serve children, adolescents, and families within our School-Based Health Centers. This is a rewarding role ideal for a licensed clinician eager to make a lasting impact on the lives of underserved communities through integrated care.
Key Responsibilities- Deliver brief, solution-focused assessments and interventions aimed at functional restoration.
- Collaborate closely with primary care providers to develop and implement clear treatment plans.
- Provide behavioral health services, including crisis intervention and substance abuse screenings.
- Educate and consult with the primary care team on mental health, chemical dependency, and behavioral medicine.
- Support patients and families with self-management, behavior change, and harm reduction strategies.
- Identify referral resources and coordinate care for patients requiring specialized services.
- Maintain thorough documentation in Electronic Medical Records (EMR).
- Conduct comprehensive bio-psychosocial assessments addressing mental health, substance use, co-occurring disorders, and medical needs.
- Manage psychosocial aspects of chronic and acute diseases in collaboration with the primary care team.
- Be immediately available during working hours to assist with crisis interventions and psychiatric emergencies.
- Provide individual, group, and family interventions as needed within the school setting.
- Use electronic tracking tools to monitor treatment outcomes and ensure effective care coordination.
- Educate staff, patients, and families on mental health best practices.
- Master’s degree in Clinical Social Work, Counseling, or Psychology.
- Licensed in the State of Ohio (LISW, LPC, LPCC, LMFT).
- Three (3) or more years of experience in behavioral health, preferably in an integrated primary care setting.
- Strong knowledge of population-based healthcare, behavioral medicine, and evidence-based treatment models.
- Experience working with diverse, multicultural populations.
- Bilingual (Spanish/English) strongly preferred.
