- Work in the community with individuals who have been assessed and are identified to need further treatment related to their substance abuse.
- Develop competency in providing trauma informed and diversity awareness advocacy.
- Work to engage in a relationship with consumers, encouraging hope and working towards achieving personal recovery goals.
- Model personal responsibility for recovery, and engage in individualized activities that promote recovery, well‑being, and independence.
- Work to ensure the client maintains a connection with a provider while working towards their recovery.
- Collaborate with system partners, providers, and other invested individuals.
- Must be in active recovery and maintenance.
- High school diploma or GED.
- Case management experience providing community outreach and engagement with traditionally resistant populations.
- Relevant knowledge of motivational interviewing, mental health and in recovery from substance misuse.
- Must be eligible to bill Medicaid.
