National Church Residences is seeking a Transition Coordinator to support adults moving from long-term care facilities to home or community settings in the Cincinnati, OH area. This is a Monday–Friday in-person role serving Butler, Clermont, Clinton, Hamilton, Warren, and adjacent counties.
You will coordinate services with case managers, arrange housing options, and ensure smooth, person-centered transitions following organizational policies and regulatory requirements.
