Aventura at West Park is seeking a compassionate and motivated PRN Occupational Therapist (OT) to join our dynamic in-house rehabilitation team. If you're passionate about helping patients regain independence and improve their quality of life, we'd love to hear from you!

• PRN (as-needed) schedule with flexible availability
• In-house therapy program
• Skilled Nursing & Long-Term Care setting
• Collaborative interdisciplinary team
• Supportive leadership and positive work environment
• Opportunity to pick up additional hours as available

• Evaluate residents and develop individualized treatment plans
• Provide skilled occupational therapy services to improve functional independence
• Collaborate with physicians, nursing staff, therapy team members, residents, and families
• Maintain timely and accurate documentation in Net Health
• Utilize Point Click Care  (PCC) as part of the interdisciplinary care process
• Ensure compliance with all state, federal, and company regulations

• Current Ohio Occupational Therapist (OT) license in good standing
• Graduate of an accredited Occupational Therapy program
• Skilled nursing or long-term care experience preferred, but not required
• Strong communication, organization, and clinical skills
• Experience with Net Health and Point Click Care  (PCC) is preferred
