The Community Liaison conducts and supervises the long-term care admissions process in order to achieve and exceed occupancy goals for the life plan community in accordance with company standards and federal, state, and local standards, guidelines, and regulations.
Essential Activities and Tasks Administration and Reporting - 50%Exercises discretion and independent judgment with respect to matters of significance in relation to admissions.
Facilitates pre-certifications and re-certifications for insurance patients.
Acts as initial point of contact for most of the patient records coming into the life plan community.
Collects detailed information to manage active referrals and to generate data to review performance statistics.
Maintains records of admissions and referrals.
Willingly and consistently shares observations and insights of departmental operations that may improve quality and efficiencies of department.
Attends all-employee and departmental meetings and in-service training as scheduled or as directed.
Grows the referral source pool and manages referral sources including informing them of services offered, strengths, accomplishments, and availability of the life plan community.
Attends events to network with referral sources which may be in the early morning or evening.
Handles referrals appropriately that the life plan community is not able to take.
Visits hospitals to meet with residents and prospects and prior to new patient or resident admission as necessary.
Manages communication between the clinical team and the insurance company.
Conducts tours of the life plan community.
Maintains the confidentiality of prospect, patient, and resident information.
Provides the Corporate Finance Department with pre-certifications and other documentation for billing purposes.
Assesses information for potential residents regarding ability to pay and cost containment.
Proactively seeks out updated information from the residents’ charts and from the nursing staff to discuss with insurance companies.
Ensures contracts and admissions paperwork are signed upon admission.
Fosters, cultivates, and maintains positive relationships with the residents and their families.
Provides timely and clear information to prospective residents, patients, family members, staff members, and third-party payers.

Monitors accountability with organization requirements prescribed by the Employee Handbook. Documents and engages employees who are routinely not observing these requirements in an effort to achieve compliance. Works with Human Resources when circumstances arise that necessitate further documentation and intervention.
Participates in the development, planning, scheduling, conducting, and monitoring of in-service training programs, on-the-job training, and orientation programs for departmental staff.
An associate degree in a related field is required.
Bachelor’s degree in marketing or a related field preferred.
Two years of long-term care admissions experience preferred.
Three years of experience in a long-term care, hospital, or other clinical setting required.
Experience in sales and marketing preferred.
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet is required.
Must be able to read, write, speak, and understand the English language.
Must possess a valid driver's license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
