Job Description

This change reflects our continued commitment to providing comprehensive, person-centered care that addresses the full range of behavioral health needs for Ohioans. All positions and services now fall under the Ohio Department of Behavioral Health as we move forward in serving individuals, families, and communities across the state.

What you'll do at DBH Treatment Planning and Clinical Services Develop, implement, and regularly review individualized treatment plans with the patients and interdisciplinary team Monitor patient progress and modify interventions based on clinical needs and treatment response Document clinical services assessments, interventions, and patient progress accurately and timely Identify patient strengths, treatment needs, risk factors, barriers to discharge and available support Forensic and Legal Responsibility Collaborate with legal representatives, courts, attorneys, probation/parole personnel and other authorized agencies as appropriate Assist with evaluations and treatment activities related to competency restoration, criminal responsibility, or other forensic treatment requirements Maintain awareness of court orders, legal requirements and patient rights affecting treatment and discharge planning Discharge Planning Develop and sustain collaborative relationships with external facilities, agencies, and support services to facilitate referrals and ensure effective follow up.

Assess housing, transportation, financial, employment, healthcare and community support needs Coordinate referrals to outpatient mental health services, residential programs, community providers and other resources This is an hourly position covered by the Ohio Health Care SEIU/1199 bargaining unit (union), with a pay range of 11 on the Ohio Health Care SEIU/1199 Pay Range Schedule. Normal working hours are 8am-4:30pm (Full Time - 40 hours/week.
