Job OverviewEvaluates patients, plans for and provides therapeutic interventions in a transdisciplinary/team approach. Responsible for the communication and coordination of treatment programs with other team members including the patient/family. Directly supervises assistants, interns and other support personnel, participates in program developments, continuous improvement and clinical education; and provides in-services to staff, hospital personnel and the community. Will also be expected to be a member of a collaborative team which is patient focused, functional outcome based and patient/family centered.
Job Requirements- Bachelor's Degree
- Basic Life Support for Healthcare Providers (BLS)
- State Licensure
- Up to 1 year experience in a related field
- Connecting with and monitoring fall prevention process in physician practices via Epic
- Education and training with Physician practice staff
- Triaging high risk fall referrals from physicians
- Providing follow up phone calls to high risk fall patients
- Providing connection to community education programs as indicated
- Completing Occupational Therapy fall prevention home visits for grant funded program as indicated.
- Gathering fall prevention data from Epic for grant reports and internal purposes as indicated.
- Deliver Evidence Based Fall prevention programs (either Stepping On or Tai Chi for Health), once training and certification is completed.
