The Christ Hospital Home Health Care
Schedule:
PRN 1 day per week (8 AM - 5 PM)

As a Home Health Aide, you’ll provide one-on-one care, build lasting relationships with patients and families, and be part of a supportive clinical team dedicated to improving lives every day.

- Observe and report changes in patient condition
- Communicate with nurses, patients, and families
- Complete required documentation
- STNA, CNA, or 75-hour Nurse Aide Training
- 1+ year of caregiving experience
- Valid driver's license and auto insurance
- Compassionate, dependable, and patient-focused

We start at the facility bedside and coordinate each patient’s transition back to their home, ensuring a smooth transition and that the care plan is right for them. We support complex, high-acuity, medically-at-risk aging patients. This is the most challenging group of individuals to serve, and we find it the most rewarding. And we do this all in partnership with health systems.
