The University of Cincinnati seeks a Licensed Independent Social Worker to provide advanced clinical social work services within our Healthcare & Medical Services department. The LISW will conduct psychosocial assessments, deliver individual and family counseling, coordinate discharge planning, and connect patients with community resources. Working in UC's inclusive, collaborative environment, this role partners with interdisciplinary healthcare teams to address social determinants of health, support complex care needs, and enhance patient outcomes.

The position requires strong clinical judgment, excellent communication, and commitment to ethical, patient-centered care.

Responsibilities

  • Provide psychosocial assessments, counseling, and care planning for patients and families in a healthcare setting
  • Coordinate discharge planning, community referrals, and resource linkage to support continuity of care
  • Collaborate with interdisciplinary teams, including physicians, nurses, and researchers, to optimize patient outcomes
  • Maintain accurate clinical documentation in accordance with legal, ethical, and institutional standards
  • Advocate for patients, addressing social determinants of health and barriers to care
  • Participate in quality improvement initiatives, case conferences, and program development
  • Provide crisis intervention, risk assessments, and safety planning as needed
  • Educate patients and families on treatment options, coping strategies, and available support services
