Tri Health Hospice in Cincinnati is seeking a Social Worker to support patients and families in long-term care settings. You will assess psychosocial needs, develop comprehensive care plans, and collaborate with the multidisciplinary team to ensure smooth transitions across care levels.
The role is Monday–Friday days, with on-call coverage including some weekends and holidays. A MSW, Ohio licensure, and up to 1 year of clinical healthcare or community social work are required.
