Transfer Center RN

The Transfer Center RN serves as a dedicated clinical coordinator/liaison responsible for managing and facilitating non-emergent outbound patient transfers to external hospitals and discharges. This role provides oversight of the transfer process from the point of transfer determination through successful transition of care, ensuring safe, efficient, and patient-centered coordination among multidisciplinary teams, receiving facilities, patients, and families.

Hours:

Monday to Friday 10a-6pm role; 1.0 FTE (40hrs)

• BSN from ACEN, CCNE or NLN CNEA accredited institution or MSN required.
• 5+ years related experience.
• Active Ohio RN License. May be required to obtain other state licensure.
• Active certification and some require specialty certification.

• Master's degree (or higher) in Nursing.
• 8+ years related experience.
• Active Ohio RN License. May be required to obtain other state licensure.
• Active certification and some require specialty certification.
