- Ability to engage children/youth/adults in care.
- Provide case coordination and care management for assigned clients.
- Document services accurately in accordance with Medicaid and agency standards.
- Participate in care planning and multidisciplinary meetings.
- Collaborate with identified sources and service providers to complete timely referrals.
- Maintain current knowledge of community resources that address the needs of individuals served.
- Monitor client progress and reassess level of care needs.
- Identify and resolve barriers to treatment engagement.
- Maintain regular communications with stakeholders to coordinate services.
- Adovocate for client access to appropriate services and supports.
- Serve as a connector to services based upon identified diagnostic assessments.
- Support continunity of care across systems (behavioral health, school, child welfare, medical).
- Perform other duties as assigned.
- Bachelor's degree in social work, counseing, or related behavioral health field.
- Experience working with children with behavioral health needs.
- Valid driver's license, auto insurance and reliable transportation (if applicable)
- Ohio license in either social work or counseling preferred.
