Central Clinic Behavioral Health in Cincinnati, OH seeks a care coordinator to engage clients across children, youth and adults, coordinate services, and support multidisciplinary care planning.
Responsibilities include documenting services per Medicaid standards, coordinating referrals, maintaining knowledge of community resources, and advocating for client access to appropriate services. A Bachelor’s degree in social work, counseling, or related field is required;
Ohio license preferred.
