As a Licensed Social Worker (LSW), you will play a vital role in our skilled nursing and long-term care team, serving the Cincinnati, OH area. - Assess and address residents’ psychosocial, emotional, and mental health needs
- Complete admissions, care plans, and discharge planning documentation
- Collaborate with nursing, therapy, and leadership teams to support resident outcomes
- Advocate for resident rights, dignity, and individualized care planning
- Provide counseling and support to residents and families
- Coordinate and maintain community resources and services
- Ensure timely, accurate documentation in compliance with regulatory standards
- Support implementation of policies related to social services and resident care
- Active LSW license in the State of Ohio (REQUIRED)
- Minimum 2 years of social work experience in long-term care or skilled nursing
- Strong communication, organizational, and interpersonal skills
- Ability to work effectively in a fast-paced, team-oriented environment
- Passion for working with geriatric populations and advocating for resident well-being
