Community Health Worker

The Community Health Worker will act as the liaison and advocate between UC Health patients and "medical homes", community-based healthcare centers, or community providers to advocate for patients through educating, mentoring, coaching and outreach. The primary goal of the Community Healthcare Worker will be to identify patients who are in need of primary care, "medical home" needs, community resources and care coordination to promote basic healthcare access and meet social, physical and developmental needs for a diverse group of patient populations.

Responsibilities include engaging in population appropriate communication, having knowledge of growth and development milestones and tasks, giving clear instructions to patients/family regarding treatment plans, involving family/guardian in the assessment, initial treatment and continuing care of the patient, identifying any physical limitations of the patient and deploying intervention when necessary, recognizing and responding appropriately to patients/families with behavioral health problems, interpreting population related data and plans the care appropriately, identifying and responding appropriately to different needs resulting from, unique psychological needs or those associated with religious / cultural norms.

Performs treatments, administers medication or operates equipment safely, recognizing and responding to signs/symptoms of abuse or neglect.

Additional responsibilities include identifying patients who are in need of "medical home" services or community-based health care services, collaborating with specialty care teams and proactively monitoring tracking system to identify patients who are in need of "medical home" services and healthcare access education, providing information, education and resources to patients to assist them in overcoming barriers to accessing healthcare and in following through with scheduled visits, setting appointments for patients with community "medical homes", health service providers and UC Health outpatient services, providing informal counseling, social support, and culturally appropriate health education, contacting patients for appointment reminders, acting as advocate for patients and families with community "medical homes: and health service providers, following up with patients and providers after scheduled appointments, facilitating conflict resolution, building and fostering relationships and collaborative with community "medical homes" and other community-based health service providers, collaborating with UC Health clinical departments and ancillary services to ensure efficient patient transition to the outpatient clinics, maintaining an updated database of community "medical home" facilities, maintaining database of outreach work and referred patients, generating database to measure community healthcare programs productivity, progress, and success, benchmarking internal processes to improve practice and demonstrate efficiency through appropriate use of resources and metric management through data.

Required skills and knowledge include demonstrating effective problem solving/decision making abilities, effective communicator with ability to speak to large groups as well as individuals from all backgrounds (both in person and over the phone), ability to communicate with clinical staff with basic medical terminology, able to work in fast paced environment where conditions can change rapidly, working knowledge of local community-based healthcare networks, working knowledge of general computer operating systems, specifically Microsoft Office programs.

(Outlook, Excel, and Power Point), working knowledge of accepted ethical standards and HIPPA within the medical community and professional organization, ability to work independently or with little supervision, strong organization skills, bilingual in Spanish preferred.

Work environment includes working in a normal office environment, travel to the various hospitals and work locations will be necessary, work hours will vary from time to time depending upon the needs of the business, regular and predictable on-site attendance is required for this position.

License & Certification:
Ohio Certified Community Health Worker Certification (CCHW) required. Recently certified, licensed or graduates within past 12 months. Minimum

Required:

0 - 6 Months equivalent experience. Preferred: 7 - 11 Months equivalent experience.
