Overview
The Nurse Practitioners believe in excellence and are passionate about providing outstanding patient care and promoting wellness services as the premier provider of quality, accessible, and convenient care. The Nurse Practitioner will be responsible for providing patient care consistent with Healthcare onsite clinic scope of services for patients 18 months or older. Our professional clinics are located within a community Onsite Clinic Walgreens store and/or Miami University and staffed 363 days a year.
Nurse Practitioners are flexible in staffing the onsite clinics and float the market as well as adjust schedules to meet market and patient needs. Nurse Practitioners are the heart and soul of our nurse practitioner-based health care model committed to providing and promoting patient-centered communication and care. The Nurse Practitioner will have a rotating work schedule to meet the staffing needs of the individual clinics.
Clinics are open 7 days a week, 363 days a year and staffed each day by our Providers.
- Master s Degree in Nursing
- Graduate from NP, CNS or midwife program
- Board Certified APRN
- Specialty certification in area of expertise, Registered Nurse
- Demonstrated expertise in area of clinical specialization
- Professional membership in specialty nursing organization required within one year of start date
- 3-4 years experience Clinical Nursing Nursing practice in Family practice, Geriatric, Occupational health, or other applicable setting
- Provides episodic and chronic care, screening, and education to Healthcare Clinic patients, and works cooperatively with wellness service colleagues.
- Supports and follows the most current evidence-based guidelines and provides diagnostic screenings and common vaccinations.
- Participates in ongoing Quality Assurance audits to include Peer Review, CP Review, Medication Administration review, and continuing education, and provides feedback to leadership.
- Achieves company objectives related to healthcare quality, patient engagement, and finances, and advocates clinic services.
- Participates in ongoing professional education, at the national, state, and local Nurse Practitioner organizations.
- Provides patient care consistent with Tri Health Clinic s expectations for patients 18 months or older.
- Nurse Practitioners are flexible in staffing the clinics and float the market as well as adjust schedules to meet market and patient needs.
- Proficient in patient registration and copay collection process and works independently primarily as a solo provider without clerical or clinical support.
- Collaborates and consults with the patient and family when diagnosing, prioritizing problems and implementing interventions; initiates and promotes collaboration to enhance the onsite clinic environment of patient care.
- Assists patients and families in selecting therapies that integrate perspectives of cost and quality of benefits.
- Collaborates with other disciplines in teaching, consultation, management, and research activities to improve outcomes in nursing practice and healthcare in the Onsite Clinic environment.
- Completes follow-up calls in 24-48 hours per departmental policy.
- Provides leadership in the clinic setting and the profession; develops innovative solutions for patient care problems and participates in healthcare-related decision-making at system, local, state and national levels with emphasis on patient outcomes.
- Promotes best practice by providing leadership in the development and application of consensus-derived protocols or evidence-based practice (EBP) guidelines.
- Provides education to patients, families, and healthcare providers to promote best practice; serves as a teacher, mentor, and role model to nurses in the promotion of nursing and EBP.
- Teaches, coaches and supports nurses and other healthcare professionals to advance patient care; collaborates in assessment, development, implementation, and evaluation of patient and staff educational needs.
- At least 50% of position involves direct management of the patient s and/or family s care; independently performs and documents a complete history and physical examination, including psychosocial assessment.
- Prescribes drugs and therapeutic devices consistent with the formulary; assesses for interactions and synergy; orders laboratory and radiologic tests and interprets results.
- Collaborates with physicians to plan and implement a comprehensive plan of care; discusses patient s care with family members; performs duties per the Standard Care Arrangement.
