Home Health Agency Field Nurse – Registered Nurse II/III/IV
Join Cincinnati Children’s as a Field Nurse supporting home‑health patients across the Cincinnati area. This part‑time position requires a strong clinical judgment and interprofessional collaboration to deliver high‑quality, family‑centered care.
Responsibilities- Assess patient needs and advance care plans by integrating age‑specific and culturally inclusive concepts.
- Collaborate with interdisciplinary teams and community resources to coordinate care and improve documentation of outcomes.
- Advocate for patients and families, facilitating shared decision making and ensuring a healing environment.
- Maintain current regulatory compliance, safety policies, and certifications, leading charge nurse duties as required.
- BSN from an accredited institution or MSN required; MSN preferred.
- 5+ years of related home‑health or acute‑care nursing experience.>
- Active Ohio RN license; other state licenses acceptable.
- Active specialty certification required or highly preferred.
Lincoln Building – Part‑time (0.6 FTE), 24 hrs per week, rotating shift.
