Job DescriptionStep into a meaningful role within our Hospice Care Team as a Registered Nurse dedicated to supporting patients and families during some of life’s most vulnerable moments. LocationHospice of Cincinnati – Requires travel. This position involves driving to patients’ locations for SIA visits. SIA (Service Intensity Add‑on) visits provide additional care during the last seven days of a patient’s life, offering more intensive and focused care during final stages.
Work Schedule- Part Time 8am‑8:30pm
- Day Shift – Weekend Only (Saturday and Sunday)

Job OverviewThis position applies the nursing process in providing direct and indirect holistic care to patients and their families. It collaborates collegially with physicians and other health care team members to meet patient needs. The role may delegate certain aspects of care to staff within scope of practice and requires weekend hours with potential on‑call evenings and nights, demanding flexibility.
Job Requirements- Associate’s Degree or Diploma in Nursing
- New hires must obtain BSN within 5 years of hire
- Basic Life Support for Healthcare Providers (BLS)
- Registered Nurse
- Knowledge, judgment, and skills related to principles of biological, physical, behavioral, social, and nursing sciences to meet complex health care needs across the life cycle
- Preferred membership in a related professional organization
- Up to 1 year experience in clinical nursing unit‑specific area
- Perform initial and ongoing assessment of patient and family; complete assessment tool; document per unit/Tri Health guidelines; communicate findings; include health counseling and teaching needs.
- Plan care based on assessment and standards of care; initiate and individualize care guidelines/plan; update plan as needed; develop short and long‑term goals with patient/family and team; communicate plan.
- Provide a safe therapeutic environment; maintain patient autonomy, dignity, and rights; seek resources for ethical decisions; balance patient and unit needs; recognize emergencies and act accordingly; document within scheduled timeframe; base interventions on data; train/educate staff; serve as expertise; communicate surgical case information; carry out physician orders.
- Evaluate care plan for optimal outcomes; document patient/family response; collaborate with patient/family and team to revise plan; supervise delegated care.
- Perform technical skills per policy and accepted standards; safely administer medications and treatments; monitor effects; use equipment safely.
- Assume organizational and educational responsibilities; identify professional and self‑improvement areas; support continuing education of team; share knowledge; provide constructive feedback; maintain awareness of continuous improvement changes.
