An Advanced Practice Provider is a licensed and board‑certified nurse practitioner or physician’s assistant specifically assigned to a Longevity territory for the purpose of comprehensive assessment and compliant documentation of the members’ conditions.
The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Statement of
Purpose:
To improve the health and quality of life of nursing home residents by caring for their unique medical, social, and emotional needs.
Travel: 75% of time in assigned territory.
Key Tasks and Responsibilities- Conduct routine and as‑needed member assessments.
- Perform comprehensive assessments, physical exams, and chart reviews to evaluate and manage the medical needs of patients.
- Accurately and thoroughly document patient encounters, including diagnoses, treatments, and outcomes in compliance with organizational and regulatory standards.
- Identify quality care and immunization gaps and notify local LHP care team. Collaborate with local LHP care team, primary care physicians, interdisciplinary care teams, including facility staff, to ensure continuity and coordination of care.
- Communicate effectively with patients, families, and caregivers to explain care plans and address concerns.
- Participate in quality improvement initiatives and data tracking related to member outcomes and performance metrics.
- Ensure compliance with all applicable healthcare regulations and documentation standards.
- Respond to documentation queries within 7 business days.
- Promote Longevity Health with members, loved ones, and nursing facilities and other stakeholders.
- Function autonomously and manage time independently to prioritize role responsibilities.
- Be member‑centric and customer‑service oriented, establishing meaningful relationships that ensure effective and high‑quality care for Longevity Health members.
- Understand conditions common to the long‑term care population and use evidence‑based guidelines and experience to make effective decisions.
- Identify and respond to potential patient changes in condition.
- Document member care in the Electronic Medical Records timely and accurately.
- Serve as documentation subject matter expert for clinical colleagues in the territory.
- Understand health plan benefits and services.
- Understand the nursing home regulatory environment.
- Communicate effectively verbally and in writing.
- Resolve conflicts / overcome barriers in an effective and appropriate manner.
- Maintain licenses, board certifications, DEA, and annual requirements.
- Adhere to all Longevity Health Policies, Procedures and Guidelines.
- Maintain compliance with Medicare, HIPAA, PHI and Cyber Security rules, laws.
- Attend meetings and educational sessions.
- Display a respectful, collaborative team approach to colleagues.
- Perform other related duties as assigned.
- Supervisory Responsibilities:
There are no supervisory responsibilities for this position.
- Active, state‑unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required. Will consider acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state‑unrestricted licensed Physician’s Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
- Active DEA License registered in the state of practice required.
- Must have an active national provider number (NPI) and an active Medicare provider number.
- Must be eligible to participate in the Medicare program as a provider and registered.
- Valid state driver’s license with a good driving record and proof of automobile insurance required.
- Auto insurance coverage as required by your state of residence. New employees will be responsible for providing proof of coverage prior to hire and on an ongoing basis.
- Must be willing to travel within a market as assigned.
- There may be requests to obtain additional state licensure once in the role.
