Look no further than The Home at Hearthstone! We are currently looking for compassionate Nurses (LPN/RN) to join our team! We have openings for Full and Part Time Night Shift.

The Home at Hearthstone is a 96 bed facility located in Mt. Healthy. We offer short term, long term and memory care to our Resident's. Our nursing assistants work a schedule of 6P-6A and rotate weekends and holidays. No more than 16 Residents per STNA with the normal being 10-12. Clean facility with adequate supplies and staffing ratios!

RESPONSIBILITIES

AND MAJOR DUTIES:
  • Ensure accurate and timely administration and distribution of all medications to assigned residents within facility.
  • Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided. Follow established standards of nursing practices and implement facility policies and procedures.
  • Obtain report from and provide report to nurse being relieved or coming on duty and record/relay sufficient information to implement appropriate follow‑up action as necessary.
  • Assess and properly document comprehensive assessment of residents as assigned.
  • Develop and implement an accurate comprehensive care plan based on each resident’s needs and his/her comprehensive assessment.
  • Identify resident problems and emergency situations and initiate emergency care and lifesaving measures in the absence of a physician.
  • Provide supervision to Nursing Assistant providing direct resident care.
  • Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service when necessary.
  • Participate in the employee evaluation process by providing information on the performance of Nursing Assistants when requested.
  • Promptly report changes in residents’ conditions to the physician, Director of Nursing Service and responsible party, and take follow‑up action as necessary.
  • Order, receive and store medications appropriately.
  • Document resident progress notes as required.
  • Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers.
  • Obtain continuing education required to maintain licensure.
  • Observe infection control and safety procedures.
  • Attend and participate in all scheduled staff meetings, educational meetings/in‑services and all other meetings related to employment.
  • Assist with and participate in all resident activities to facilitate a home‑like environment for the residents within assigned facility.
  • Ensure compliance with all HIPAA regulations.
  • Respectfully engage and welcome all visitors, guests and family members within the facility.
  • Ensure a safe and sanitary living environment for residents.
  • Adhere to all infection control policies within the assigned facility.
  • Ensure the Resident Assessment Instrument (RAI) process is followed in accordance to State & Federal Guidelines.
  • Follow Residents’ Rights policies at all times.
  • Adhere to all facility policies & procedures.
  • Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all times.
  • Perform other related duties as directed by Licensed Nurse and/or person of authority.
