Schedule:
Monday - Friday, 8:15AM – 5:00PM with on‑call requirements

- Licensed Practical Nurse with a current license in the state of employment.
- Minimum one year of experience as an LPN in an acute care setting; two years preferred.
- Home‑care experience desired.
- Effective communication skills and the ability to create positive impressions with patients, families, physicians and co‑workers.
- Calm, patient, and accommodating demeanor.
- Compassionate and caring while working with patients.
- Knowledgeable in nursing best practices.
- Ability to make appropriate nursing judgments.

- Valid driver’s license and auto insurance with your name listed as a driver.
- Provides skilled nursing care in a patient’s home as ordered by the attending physician.
- Performs care by following order sets and goals made by an RN.
- Observes and monitors patient conditions.
- Notifies RNCM of any changes in patient condition.
- Administers medication as prescribed by the physician.
- Counsels patient and family on disease/injury processes and how to manage.
- Incorporates patient and family in development of plan of care.
- Helps decrease read‑hospitalizations by teaching patients preventative measures and making good decisions.
- Listens to patient and family members.
- Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
- Documents all aspects of treatment, assessments, and patient education.
- Maintains an active LPN license in the state of employment.
- Communicates with scheduler about any changes outside the normal schedule.
- Participates in all on‑call requirements and case conferences.
- Attends in‑service trainings and mandatory agency meetings.
- Maintains a professional appearance as a representative of the company.
- Understands and practices agency policies and procedures.
- Other duties as assigned.
- Drives to patient’s primary location per scheduled visit.
- Daily attendance at assigned visit locations.
- Documents all aspects of subsequent visits within 24 hours to meet Medicare guidelines.
- Completes and submits all required documentation within specified company requirements.
- Knowledgeable with providing skilled care within the scope of practice for an LPN.
- Fulfills LPN visits in assigned geographic location per patient need within federal and state specifications.
- Reading, speaking, writing English communication skills, computer/PDA usage.
- Hand/finger dexterity; hearing/seeing; talking in person; hearing in person; vision for close work.
- Walking, sitting, driving, talking on the phone.
- Bending, standing, lifting up to 50lbs. with or without assistance; stretching/reaching; climbing; stooping.
- Distinguish smell/taste rarely.
