The Christ Hospital Home Health Care schedule is Monday–Friday, 8:15 AM–5:00 PM.

• Provide skilled nursing care in patients’ homes under physician orders.
• Follow care plans and goals created by the RN Case Manager.
• Monitor patient conditions and report changes.
• Administer medications and treatments as prescribed.
• Educate patients and families on disease management and preventative care.
• Help reduce re‑hospitalizations through patient education and proactive care.
• Complete clinical documentation in accordance with agency and Medicare guidelines.
• Participate in on‑call, case conferences, trainings, and required meetings.

• Current Licensed Practice Nurse license in the state of employment.
• Minimum of 1 year of LPN experience in an acute care setting; 2+ years preferred.
• Home health experience preferred.
• Valid driver’s license and active auto insurance.
• Strong communication, time‑management, and documentation skills.
• Compassionate, dependable, and patient‑focused.
