The Licensed Practical Nurse (LPN) is responsible for providing safe and effective nursing care to residents in accordance with established care plans. The LPN works collaboratively with physicians, nursing leadership, and interdisciplinary team members to ensure quality resident outcomes.

• Provide direct nursing care to residents

• Administer medications and treatments as ordered

• Complete nursing assessments and documentation

• Perform wound care and monitor resident conditions

• Supervise and support Nursing Assistants

• Communicate changes in resident condition to appropriate parties

• Ensure compliance with all state, federal, and facility regulations

• Graduate of an accredited nursing program

• Current, active, and unencumbered LPN license

• Current CPR certification

• Knowledge of long‑term care nursing practices

• Strong communication and teamwork skills

• Ability to work effectively with residents, families, and staff
