As a Home Health LPN, you’ll provide one-on-one patient care, support recovery, educate patients and families, and work alongside a collaborative clinical team focused on positive patient outcomes.

** What You’ll Do
*** Provide skilled nursing care in patients’ homes under physician orders
* Follow care plans and goals created by the RN Case Manager
* Monitor patient conditions and report changes
* Administer medications and treatments as prescribed
* Educate patients and families on disease management and preventative care
* Help reduce re-hospitalizations through patient education and proactive care
* Complete clinical documentation in accordance with agency and Medicare guidelines
* Participate in on-call, case conferences, trainings, and required meetings
** Qualifications
* ** Current Licensed Practical Nurse license in the state of employment
* Minimum of 1 year of LPN experience in an acute care setting required
* 2+ years preferred
* Home health experience preferred
* Valid driver’s license and active auto insurance
* Strong communication, time management, and documentation skills
* Compassionate, dependable, and patient-focused
