Sanctuary Hospice is seeking a Sanctuary Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.

In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. - Respond to clinical referral information with care and expertise, addressing inquiries and requests.
- Partner closely with the Clinical Director to assess eligibility and suitability of clients for hospice services.
- Coordinate care, ensuring patients’ comfort and quality of life remain paramount.
- Deliver top‑notch nursing care rooted in proven principles and techniques.
- Educate and advocate for patients and families, encouraging participation in personalized care plans.
- Participate in call duties, providing support when needed.
- Hold current unencumbered RN license.
- Minimum one year experience as an RN in a medical‑surgical/acute care setting; hospice experience a plus.
- Computer proficient in typing and various programs, including EMR.
- Possess current CPR certification if required by state.
- Current driver’s license, vehicle insurance, and dependable transportation.
$38.00 – $44.00 per hour (average pay range). The pay range listed is a general guideline and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and specific responsibilities of the position and will be confirmed at interview conclusion.
