40 (4-10 hour shifts)

The Oncology Nurse Navigator is an RN who provides individual assistance to patients, families, and caregivers to overcome health‑care system barriers and facilitate timely access to quality care within a community oncology setting. Utilizing the nursing process, the Navigator offers patient education, ensures access to internal and external resources, supports informed decision‑making, and promotes high‑quality health and psychosocial care throughout the cancer continuum.

The role involves frequent collaboration with internal departments and external referral resources, and regular interface with practice‑based leadership teams, physicians, and clinical staff to advance the patient navigation process.

Responsibilities
  • Coordinate care throughout the full continuum of cancer care experience.
  • Identify potential and real gaps in care delivery; seek opportunities for avoidance or closure.
  • Prepare for, organize, and lead the Patient Care Conference (huddle) at regular intervals; communicate interventional outcomes to the full care team promptly.
  • Assess patient risk stratification.
  • Conduct basic psychosocial assessments and provide emotional support; delegate or refer for additional screening and support as needed.
  • Develop appropriate nursing diagnoses and corresponding action plans when needed.
  • Support and/or provide oral chemotherapy education, adherence monitoring, and side‑effect management.
  • Participate in survivorship and advance care planning visits; make referrals as needed.
  • Participate in research, quality, and performance improvement activities, including reporting on navigation metrics.
  • Support patient advocacy and empowerment.
  • Participate in community outreach and cancer prevention education.
  • Maintain knowledge of practice operations guidelines, organization, and health‑care economics.
Qualifications

Minimum Requirements:

  • Bachelor’s prepared Registered Nurse (BSN) preferred;
    Oncology Certification (OCN) preferred;
    Nurse Navigator Certification a plus.
  • Three years’ work experience as a Registered Nurse in an oncology practice or clinic.

  • Demonstrated leadership, interpersonal, and collaboration skills.
  • Ability to apply the full nursing process in day‑to‑day clinical operations, including assessment, analysis, nursing diagnosis, care planning, implementation, and outcomes evaluation.
  • Ability to assess health disparities, identify needs of populations and cultures within the service area, describe a patient navigation process to address those needs, and document outcomes.
  • Ability to prioritize and re‑prioritize quickly.
  • Strong organizational and critical thinking skills.
  • Ability to apply and/or oversee care coordination in conjunction with the multidisciplinary team.
  • Strong verbal and written communication skills.

Additional Knowledge/

Skills:

  • Working computer and internet skills, including an electronic EMR system.
  • Basic knowledge of local community resources.

Occasionally lifts and carries items weighing up to 40 lbs. Requires corrected vision and hearing to normal range. The position requires lifting and assisting patients as requested.
