As a Post-Acute Care Coordinator Manager, you will lead a team of Post‑Acute Care Coordinators (PACCs) to assist patients in the transition of care from one setting to the next within the health system.

- Operations: Confirm complete and timely coordination of health care services and ancillary services according to physician orders.
- Hiring and Training: Select and onboard staff; provide oversight for training and orientation of PACCs, educating them about company services (physician’s offices, long‑term care facilities, rehabs, hospitals).
- Collaboration: Participate in care conferences and case management coordination; ensure all patient, hospital, and staff issues are fully resolved; collaborate with PACCs to maintain facility relationships.
- Supervision: Directly supervise all department employees and manage employee relations issues.
- Discharge Planning: Work with PACCs, hospital/facility personnel in the discharge planning process.
- Minimize Patient Risk: Promote patient well‑being as part of the facility team(s).
- Policies: Maintain current knowledge of Medicare, insurance, company, and hospital guidelines, policies, and protocols.
Compassionate communicator with a positive attitude; capable of identifying situations and providing appropriate solutions; patience in dealing with patients, families, physicians, and coworkers; meticulous attention to detail and adherence to directions.
Requirements- Registered Nursing (RN) license in the state of service
- Multi‑site supervisory experience
- Effective communication skills, high degree of creativity, and strong decision‑making ability
- Healthcare experience desirable
- Capable of meeting all physical demands of the role
