Blue Ash Health & Rehab, LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join our team in Cincinnati, OH. This role involves providing compassionate and quality nursing care to our residents, ensuring their comfort and well‑being in a supportive and collaborative environment.

• Assist in the delivery of nursing care to residents under the supervision of registered nurses and physicians.
• Monitor and document patients’ health status, including vital signs and any changes in condition.
• Administer prescribed medications and treatments accurately and safely.
• Collaborate with healthcare team members to develop and implement individualized care plans.
• Provide support and education to residents and their families regarding health and wellness.
• Maintain accurate and timely medical records in compliance with company policies and legal regulations.
• Ensure a clean, safe, and comfortable environment for residents.

• Current and valid LPN license in the state of Ohio.
• Proven experience working as an LPN in a healthcare or rehabilitation setting preferred.
• Strong knowledge of nursing practices, procedures, and standards.
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and as part of a multidisciplinary team.
