Pay:
Competitive salary, regular salary increases, potential for performance awards

The RN Case Manager assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life- threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.
Major duties may include but are not limited too:
- Coordinates care with multiple providers across all levels and sites of care.
- Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.
- Evaluates care and outcomes to ensure timely and appropriate provision of services.
- Integrates knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex situations.
- Assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization.
- Validates understanding of the rationale for care processes and decisions.
- Questions clinical practices for the purpose of providing evidence based care.
- Demonstrates the ability to cope with and manage competing priorities.
- Demonstrates competence in practice and decision-making, deliberate planning, and critical thinking skills.
Preferred Experience:
Case management and or social work experience preferred.
Supervisory Controls:
The incumbent is responsible to the assigned rating and approving supervisors and is accountable to the Associate Director of Patient Care Services (ADPCS).
