Pre-Transplant Coordinator

* Initiate, coordinate, and track pre-transplant evaluations and required testing.

* Prepare evaluation charts, review histories, and synthesize findings for committee presentation.

* Provide education and obtain informed consent for evaluation and listing.

* Collaborate with referring providers, cardiology, and multidisciplinary teams.

* Ensure accuracy and compliance of UNOS listing data, closure letters, and notifications.

* Manage evaluation communication channels (phones, faxes, imaging transfers).

* Support regulatory adherence and process improvement in evaluation workflows.
