Nurse Practitioner (Healthcare)

General Information:
An Advanced Practice Provider is a licensed and board-certified nurse practitioner or physician assistant specifically assigned to a Longevity territory for the purpose of comprehensive assessment and compliant documentation of the members conditions.

General

Purpose:

The individual holding this position is required to support and demonstrate their commitment to the Longevity Health Statement of Purpose, which is:
To improve the health and quality of life of nursing home residents by caring for their unique medical, social, and emotional needs.

The Advanced Practice Provider-Comprehensive Assessment Clinician (APP) focuses on delivering high-quality care through scheduled rounding in nursing facilities within a designated regional territory. The APP will be responsible for performing comprehensive clinical assessments, documenting member conditions thoroughly supporting quality initiatives to support optimal outcomes for our patient population. As part of the Clinical team, the APP will report to the CDI Senior Documentation managers and be a part of the CDI team.

Travel: 75% of time in assigned territory

Key Tasks and Responsibilities
  • Conduct routine and as-needed member assessments.
  • Perform comprehensive assessments, physical exams, and chart reviews to evaluate and manage the medical needs of patients.
  • Accurately and thoroughly document patient encounters, including diagnoses, treatments, and outcomes in compliance with organizational and regulatory standards.
  • Identify quality care and immunization gaps and notify local LHP care team. Collaborate with local LHP care team, primary care physicians, interdisciplinary care teams, including facility staff, to ensure continuity and coordination of care.
  • Communicate effectively with patients, families, and caregivers to explain care plans and address concerns.
  • Participate in quality improvement initiatives and data tracking related to member outcomes and performance metrics.
  • Ensure compliance with all applicable healthcare regulations and documentation standards
  • Responds to documentation queries within 7 business days
  • Promotes Longevity Health with members, loved ones, and nursing facilities and other stakeholders.
  • Functions autonomously and effectively manage time independently to prioritize role responsibilities
  • Is member-centric and customer service oriented, establishing meaningful relationships that ensure effective and high-quality care for Longevity Health members.
Job Description
  • Understands conditions common to the long-term care population and uses evidence-based guidelines and experience to make effective decisions.
  • Identifies and responds to potential patient changes in condition.
  • Documents member care in the Electronic Medical Records timely and accurately.
  • Serves as documentation subject matter expert for clinical colleagues in the territory.
  • Understands health plan benefits and services.
  • Understands the nursing home regulatory environment.
  • Effectively communicates verbally and in writing.
  • Resolves conflicts/overcomes barriers in an effective and appropriate manner.
  • Responsible for maintaining licenses, board certifications, DEA, and annual requirements.
  • Adheres to all Longevity Health Policies, Procedures and Guidelines.
  • Maintains compliance with Medicare, HIPAA, PHI and Cyber Security rules, laws.
  • Attends meetings and educational sessions.
  • Displays respectful, collaborative team approach to colleagues.
  • Performs other related duties as assigned.

Credentials & Coverage
  • Active, State unrestricted licensed Advanced Practice Nurse, National Board certification (AANP, ANCC or AACN) as an Adult, Family, Geriatric or Primary Care nurse practitioner required. Will consider acute care nurse practitioner with nursing home/geriatric or hospice palliative experience OR state unrestricted licensed Physicians Assistant NCCPA board certified with nursing home/geriatric or hospice/palliative experience.
