RN Case Manager

Role: The RN Case Manager provides end-of-life hospice care in patients' homes or community settings, supervising hospice aide staff and coordinating patient services.

• Assess, plan, implement, and evaluate patient care needs during on-call hours.
• Report activities and maintain detailed clinical documentation.
• Assist with medication and supply delivery outside regular hours.
• Participate in interdisciplinary team meetings and collaborate with physicians.
• Admit eligible patients for hospice services in coordination with the triage team.
• Develop personalized care plans addressing patient problems and goals.
• Support the use of the 4
  
  Ms (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care.

• Current RN licensure in the practicing state.
• Valid driver’s license and auto insurance.
• At least two years of RN experience, with hospice or palliative care experience preferred.
• Strong clinical judgment, excellent communication, compassion, and ability to handle sensitive situations.

The role involves working weekdays from 8 am to 5 pm, with on-call duties as needed, emphasizing a mission-driven environment focused on compassionate, patient-centered care and team collaboration.
