Full-Time MDS Coordinator – Maple Grove Nursing & Rehabilitation
Location:
Maple Grove Nursing & Rehabilitation, Cincinnati, OH

Join Maple Grove as our Full-Time MDS Coordinator! Maple Grove Nursing & Rehabilitation in Cincinnati, Ohio is seeking an experienced, detail-oriented Full-Time MDS Coordinator to oversee the resident assessment and care planning process. If you are an RN with strong assessment, documentation, and coordination skills who enjoys working behind the scenes to support quality outcomes and accurate reimbursement, this Full-Time MDS Coordinator role could be your next step.

Key Responsibilities:
  • Coordinate and complete the MDS/RAI process for all residents in accordance with state, federal, and facility guidelines.

  • Develop, update, and oversee resident-centered care plans in collaboration with the interdisciplinary team.

  • Ensure accuracy and timeliness of MDS assessments, ICD-10 coding, and related documentation that drive quality measures and reimbursement.

  • Review clinical documentation, nursing notes, therapy records, and physician orders to support accurate assessment and coding.

  • Lead or participate in care conferences, QAPI/QA meetings, and other clinical review meetings as needed.

  • Educate and support nursing and interdisciplinary staff on documentation standards related to the MDS and care planning.

  • Monitor quality indicators and proactively identify opportunities to improve resident outcomes and facility performance.

  • Maintain compliance with all CCH Corporation policies, state/federal regulations, and survey readiness related to MDS and care plans.

Qualifications:
  • Current, valid Registered Nurse (RN) license in the State of Ohio required.

  • Prior MDS Coordinator experience in long-term care strongly preferred.

  • Working knowledge of MDS, RAI process, PDPM/Medicare/Medicaid reimbursement, and care planning.

  • Strong clinical assessment, critical thinking, and organization skills.

  • Accuracy-focused with excellent attention to detail and follow-through.

  • Effective communicator and team player, comfortable collaborating with nursing, therapy, social services, and providers.

  • Proficient with electronic health records and computer-based documentation.
