• Assess, treat, and monitor residents with acute and chronic wounds
• Develop and implement individualized wound care plans and prevention strategies
• Conduct routine rounds to evaluate risk factors and ensure appropriate interventions
• Collaborate with physicians and interdisciplinary teams to coordinate care and treatment plans
• Educate and guide nursing staff on wound care protocols, prevention techniques, and best practices
• Monitor and ensure compliance with all wound care plans and regulatory requirements
• Track and report wound care data, including pressure ulcers and treatment outcomes
• Evaluate new admissions and existing residents for pressure ulcer risk and implement preventative measures
  
  Ensure proper supply levels of wound care materials and equipment
• Participate in quality improvement initiatives and support regulatory surveys
• Provide education and emotional support to residents and families

• Graduate of an accredited School of Nursing
• Current LPN or RN license in the State of Ohio
• Current CPR certification
• Minimum of 1 year of experience in wound care required
• At least 3 years of experience in healthcare or long-term care preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and collaboratively with a team
• Highly organized with strong time-management skills
• Compassionate, patient-focused approach to care
