Geriatric Pain & Palliative Care NP​/PA | New Grads | Build Practice

Join a dynamic and compassionate team as a Geriatric Pain & Palliative Care Nurse Practitioner (NP) or Physician Assistant (PA), where you will play a vital role in enhancing the quality of life for older adults facing complex pain and serious illnesses. You will provide specialized pain and palliative care to geriatric residents across skilled nursing facilities, rounding on the same patients over time rather than cycling through a rotating panel.

As the practice adds facilities across the Cincinnati area, this role grows with it.

New graduates are welcome with formal onboarding from day one. Malpractice insurance with tail coverage is included. Compensation runs $60–$100/hr based on experience plus a productivity bonus.

This is a ground-floor opportunity in the truest sense. You will help shape how this practice operates in a brand-new Ohio market, building the clinical relationships and standards that carry it forward. Pain management for older adults in skilled nursing is deeply needed and chronically under provided, and the provider who steps in here will have visible, lasting impact on the residents they follow week after week.

• Conduct comprehensive pain assessments covering medical history, physical examination, and identification of pain source and intensity
• Design individualized treatment plans combining medication management, non-pharmacological approaches, and interventional referrals for acute and chronic pain
• Prescribe and monitor pain medications in compliance with Ohio state and federal regulations
• Coordinate referrals for physical therapy, acupuncture, spinal blocks, and other interventional options
• Educate patients and families on pain control strategies and lifestyle factors
• Build and maintain strong working relationships with staff and leadership at each partner facility
• Monitor outcomes through consistent follow-up visits and refine treatment plans accordingly
• Document all care accurately in TEBRA and Point Click Care

• Active, unrestricted Ohio NP or PA license required, or eligibility to obtain
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification required
• Fully onsite across Cincinnati area skilled nursing facilities
• Comfortable traveling between multiple facility locations
• New graduates welcome, 5+ years of relevant RN experience also counts
• Prior geriatric, SNF, palliative, or hospice experience preferred
• Familiarity with TEBRA or Point Click Care  a plus

• $115,200–$192,000 based on experience
